Delivered person-centred, one-to-one complex care support to a client with quadriplegic cerebral palsy, microcephaly, scoliosis, Raynaud's syndrome, and gastrostomy needs within a home and community setting. Responsibilities included all aspects of personal care, safe manual handling using hoists, gastrostomy feeding, medication administration, and accurate daily documentation in line with care plans, risk assessments, and CQC standards. Nazmi also supported the client's emotional wellbeing and social inclusion by accompanying them to community activities, medical appointments, and cultural outings, covering both day and waking night shifts.

Provided person-centred, one-to-one complex care to a client with early-stage dementia, spinal injury, diabetes, and hypertension in a home-based setting. Duties encompassed safe medication administration via MAR charts, CPAP/non-invasive ventilation monitoring, full personal care support, mobility exercises, and preparation of diabetic-appropriate meals alongside light domestic duties. Nazmi also arranged and attended GP and hospital appointments and accompanied the client to social activities to encourage independence, dignity, and quality of life in line with CQC standards and safeguarding principles.
